From Biomarker Positivity to Clinical RA: Multi‑Autoantibody Expansion and Serological Profiling in Rheumatoid Arthritis

Rheumatoid arthritis is characterized by a prolonged preclinical phase during which loss of immunological tolerance and autoantibody diversification precede the onset of clinically apparent disease. This webinar explores the transition from isolated biomarker positivity to established RA, focusing on the concept of multi‑autoantibody expansion and its implications for serological profiling. Through a combined clinical and laboratory perspective, we will review how established markers such as RF and ACPA, together with emerging autoantibodies, inform risk stratification, diagnosis and early disease pathways. Emphasis will be placed on the latest scientific research in RA and pre-RA, assay considerations, and the opportunities—and limitations—of expanded serological profiling in current practice for both diagnosis of RA in individuals with active inflammatory arthritis, and in risk stratification for future RA.

Kevin D. Deane, MD, PhD

Dr. Deane is a Rheumatologist and Professor of Medicine at the University of Colorado, where he completed his residency and fellowship after earning his medical degree from Loma Linda University and undergraduate degree from UC Davis. He practices outpatient and inpatient clinical rheumatology at UCHealth. His research interests focus on the early development of rheumatoid arthritis (RA), particularly the preclinical phase ("pre-RA"), as well as the role of the lung in RA initiation and progression. He also serves as the Director of the Autoimmune Disease Prevention Center at the University of Colorado Anschutz Medical Campus and is a Co-Investigator on the Arthritis Foundation–funded StopRA national study.

Discuss the natural history of rheumatoid arthritis development from its 'preclinical phase' through the development of a clinical diagnosis of RA (i.e. clinical RA)

Outline the role of biomarkers, including autoantibodies, in the current prediction of future clinical RA as well as in optimizing the diagnosis of clinical RA

 

Review the landscape of clinical trials in RA prevention
